چکیده

The purpose of this study is to determine the effects of variations in cardiac output upon the mean systolic left ventricular-brachial artery gradient in patients with aortic stenosis. The magnitude of this gradient varies with valve size, flow across the valve and possibly heart rate. The effect of these parameters has been investigated in patients with mitral stenosis. " 2 Since exertional dyspnea, angina and syncope are common in physiologically significant aortic stenosis, determination of the exercise aortic gradient is of considerable interest in evaluation of the mechanism of these symptoms. In principle, exercise gradient changes may result from alterations in left ventricular systolic pressure, brachial artery systolic pressure, or both factors.

Hemodynamic effects of exercise in isolated valvular aortic stenosis.

Hemodynamic data, including simultaneously measured left ventricular and aortic pressures, were obtained from 20 patients with isolated valvular aortic stenosis at rest and during supine leg exercise. Changes in left ventricular end-diastolic pressure during exercise were not significantly related to the severity of aortic stenosis but were correlated with age; patients with abnormal left ventr...

Echocardiographic left ventricular dimensions in pressure and volume overload. Their use in assessing aortic stenosis.

Left ventricular 'relative wall thickness', determined from the ratio between echocardiographic measurements of end-systolic wall thickness and cavity transverse dimension, was related to peak systolic intraventricular pressure in 15 normal subjects, in 15 patients with left ventricular volume or pressure overload without aortic stenosis, and in 23 patients with aortic stenosis. All these patie...

Relation between symptoms and profiles of coronary artery blood flow velocities in patients with aortic valve stenosis: a study using transoesophageal Doppler echocardiography.

OBJECTIVE To analyse profiles of coronary artery flow velocity at rest in patients with aortic stenosis and to determine whether changes of the coronary artery flow velocities are related to symptoms in patients with aortic stenosis. DESIGN A prospective study investigating the significance of aortic valve area, pressure gradient across the aortic valve, systolic left ventricular wall stress ...

Three Different Imaging Modalities of a Patient with the Aortic Coarctation

The patient was a 19 year-old woman with the diagnosis of resistant hypertension, although she was under treatment of three classes of anti-hypertensive drugs (beta blocker, angiotensin receptor blocker, diuretic) for more than one year. In physical examination there was only a significant difference between the systolic blood pressure of upper and lower extremities (200 vs. 120 mmHg), without ...

Response to letters regarding article, "systemic hypertension in low-gradient severe aortic stenosis with preserved ejection fraction".

BACKGROUND Low-gradient severe aortic stenosis with preserved ejection fraction is an increasingly recognized entity, and symptomatic patients may benefit from aortic valve replacement. However, systemic hypertension frequently coexists with low-gradient severe aortic stenosis, which itself may cause elevated left ventricular (LV) filling pressures with resultant symptoms of dyspnea. METHODS ...
